[PhITEM] Withdraw your diploma

How do you withdraw your diploma?

You have three options

Pick up your diploma in person

  • Bring proof of identity
  • Go to the PhITEM training department reception desk (contact details below)

Have your diploma collected by a third party

The authorized person must go to the PhITEM training department reception desk (see contact details below) with the following documents:
  • Letter of proxy stating the identity of the person authorized to collect your diploma.
  • Photocopy of your national identity card or passport.
  • Third party's national identity card or passport.

Collect your diploma by mail order

Are you unable to attend? Send a duly completed diploma withdrawal request form and the required documents to the UFR PhITEM training service reception desk (contact details below).
